Special Church Group Event

If your committee, music group, or small group is sponsoring or hosting a special gathering or event, this form needs to be filled out specifically for that event. Be sure to check any boxes that are appropriate for special needs. Once submitted, the form goes directly to the Church Office/Facilities Manager who will process your request. You will then be contacted to verify that your request has been accepted. If you would like to check the church calendar for room availability prior to filling out the form, contact the Facility Manager in the Church office at 530-243-2403.

ie. Room 10, Social Hall, Library, etc. Your request will be confirmed by the Church Office after checking availability.
If your event is a fundraiser, please describe and give details about the event here.
